Families First of Michigan (FFM):
PCC has been providing this high-risk in-home program for over 30 years. Families First of Michigan (FFM) is an intensive, short-term, crisis intervention and family education program. It is designed to serve families that are currently considered high risk with severe needs. These families display an imminent risk for out-of-home placement of at least one child because of severe abuse, neglect, delinquency, or domestic violence. FFM also offers family reunification services to assist with a successful transition when children return home from out of home placements. Referrals come from the Department of Health and Human Services (DHHS) to include Private Agency Post adoption families. Program services will be provided regardless of family income and without cost to the client. The primary goal of FFM, whether referred by DHHS or Private Agencies for post adoption services, will be to enhance family functioning, thereby decreasing safety concerns resulting in a permanent parental placement and no further out of home placements. The program is time limited but intensive, meaning the worker is available 24 hours/day, to provide intensive services. Many program services are offered in the home, allowing staff to strengthen, empower, and preserve families, rather than removing children from their homes immediately. Referrals are made by phone to designated supervisor(s). This service is provided in Huron, Lapeer, Sanilac, St. Clair, and Tuscola Counties.
Families Together Building Solutions- Pathways to Potential (FTBS):
The Families Together Building Solutions Pathways to Potential (FTBS) program is an in-home, strength-based program working with families that are considered low to moderate risk that present multiple problems and are linked with abuse, neglect and prevention. Cases served by FTBS include CPS categories III and IV (Cat I and II need specialist permission), foster care reunification, post adoption services, and/or success coach referrals. FTBS can also serve CPS cases that closed within the last 18 months and DHHS supervised delinquency cases. Program services will be provided regardless of family income and without cost to the client. The FTBS program is a therapeutic program which focuses on concrete needs of each individual family. Services are provided to the families in-home for three hours per week for 90 days, helping families to identify solutions to their goals that will decrease safety concerns for the children/family. The service is offered with non-traditional hours and 24/7 on call crisis intervention, if needed. Referrals are made by phone to the program supervisor(s). This service is provided in Huron, Lapeer, Sanilac, St. Clair, and Tuscola Counties.
Family Reunification Program (FRP):
Family Reunification Program (FRP) is a Department of Health and Human Services (DHHS) contract in which services are provided to assist in the reunification process for children that have been in an out-of-home placement including residential treatment, foster care, relative placement, psychiatric hospitalization, detention, or non-respondent parent. Referrals are made by the DHHS and Private Agency Foster Care Workers. Program services will be provided regardless of family income and without cost to the client. FRP consists of a masters level Team Leader (or bachelors level with two years experience) who provides the therapeutic component and a bachelor’s level Family Worker who can assist with more of the family’s concrete needs. The program is four months with an occasional option to extend an additional two months. Staff are available 24/7 to families to assist with crises, and services are provided with non-traditional hours to meet the flexible schedules of families. The goal of FRP is to provide support in achieving and maintaining permanency in the family home while reducing the risk of a repeat need for out-of-home placement. Services are offered within the home and in the community to help strengthen, empower and preserve the family unit. Referrals are made by phone to the program supervisor. This service is provided in Huron, Lapeer, Sanilac, St. Clair, and Tuscola Counties.
Family Skills Family Maintenance Program:
The Family Skills Family Maintenance Program is an in-home, skills-based program which provides prevention services, family support services, and limited family reunification services. The program is designed to work with families with varied degrees of risk, serving families with CPS categories I-IV, as well as families with children in Foster Care and adjudicated youth in out-of-home placements. Services are provided in the family home or a mutually agreed upon place. The duration of this program is typically three months with an option to extend up to a year depending on needs of the family and with approval by DHHS. Services are provided with non-traditional hours to meet the flexible schedules of families. Designated DHHS workers submit a referral to their supervisor, and the contract monitor sends that referral to the program supervisor. This service is available in Huron and Tuscola Counties.

Parenting Time Supportive Services (PTSS):
Parenting Time Supportive Services (PTSS) is a contract established with DHHS in which PTSS staff supervise parenting time visits with an identified parent for those families that are currently involved with CPS or have a child in an out-of-home placement (depending which PTSS contract is used). Staff work with the identified parent and foster parent/guardian to establish an agreed upon parenting time schedule. Staff then observe the visit and interaction between the parent and child(ren). Staff members do provide feedback to the parents in regards to their parenting skills and offer suggestions as needed. Staff may provide community resource referrals to families with an identified need when appropriate. Designated DHHS workers submit a referral to their supervisor, and the contract monitor sends that referral to the program supervisor. This service is provided in Huron, Lapeer, Sanilac, St. Clair, and Tuscola Counties.
Family Assistance (FA) Program:
The Family Assistance (FA) Program is an in-home, skills-based program which works with families to provide intensive supportive interventions. The program is designed to work with families having varied degrees of risk; serving families with Categories I, II, III, families with children in foster care, juvenile delinquency and prevention services. Services are provided in the family home or in a mutually agreed upon location, for 90 days with an option to extend for an additional 90 days, with approval by DHHS. The FA Program also includes a Parenting Time Support Service component if requested by the referring worker. When Parenting Time Support is provided FA staff will also arrange and supervise court ordered parenting time visitation between the client and their child(ren.) This supervised visitation includes an educational element to increase parental abilities. Designated DHHS workers submit a referral to their supervisor, and the contract monitor sends that referral to the program supervisor. This service is provided in Sanilac and St. Clair Counties.
In-Home Family Services:
The In-Home Family Services (In-Home) Program is a skills-based program that works with families to provide moderate to intensive supportive interventions. In-Home is designed to work with families having varied degrees of risk; serving families with an active investigation or open case, families with children in out-of-home placement, families that don’t have a preponderance of evidence of child abuse/neglect but there is a risk of future harm, families inviestigated within 18 months, post adoptive services, and prevention services. Services are provided in the family home or in a mutually agreed upon location, for 180 days with an option to extend, with approval by DHHS. Designated DHHS workers submit a referral to their supervisor, and the contract monitor sends the referral to the program supervisor. This service is provided in Sanilac and St. Clair Counties.
Substance Use Disorder Family Support Program:
The Substance Use Disorder Family Support Program (SUDFSP) is designed to provide therapeutic services to families that have at least one parent substance affected. SUDFSP provides serves to cases having varied degrees of risk; serving families with an active investigation or open case, open prevention case, families with children in out-of-home placement, families that don’t have a preponderance of evidence of child abuse/neglect but there is a risk of future harm, and post adoptive services. Services are provided in the family home or in a mutually agreed upon location, for 180 days with an option to extend, with approval by DHHS. Designated DHHS workers submit a referral to their supervisor, and the contract monitor sends the referral to the program supervisor. This service is provided in Lapeer Huron, Tuscola, Sanilac, and St. Clair Counties.
